As a resident of Nevada, you should take the following steps to ensure your decision to become a donor is carried out:

• New Online donor registration form is in English and Spanish. Fill out the form, hit the "submit information" button at the bottom of the page. The form is electronically sent to the NV Donor Registry https://www.donoregistry.org/Register/Dynamic/Enrollment.aspx?RegistryOwnerID=3&LanguageCode=en


• Download, print and mail the donor registration form available at www.nvdonor.org/pdf/DonorRegistration.pdf.


• Indicate your wishes on your driver's license or state ID when you apply for or renew it.

Nevada does not require family consent to carry out your decision to be an organ, eye or tissue donor. However, keeping everyone informed will help avoid any confusion or delays.   Print a family notification card now.
